By volunteering with United Way of the Chattahoochee Valley (UWCV), I acknowledge that participation in volunteer activities involves certain risks, including injury or exposure to illness. I accept full responsibility for any such risks and related medical or other expenses. I release, indemnify, and hold harmless UWCV, its organizers, sponsors, agency partners, and activity supervisors from all claims or liabilities arising from my participation, including those caused by negligence. I understand that UWCV does not provide employment or accident insurance for volunteers. I agree to follow all written and verbal instructions provided by UWCV staff. Failure to comply may result in the revocation of my volunteer privileges and being asked to leave the premises.

I grant permission to United Way of the Chattahoochee Valley to use my likeness, story, and any photographs or media content featuring me for purposes including printed materials, videos, websites, and media releases. I understand that UWCV may edit or modify content as needed and may use it at its discretion without expectation of compensation. I further acknowledge that this permission applies to future materials without further notice. By signing below, I confirm that I have read, understood, and voluntarily agree to this release.
